If your most recent passport has been lost or stolen,

 

 

To replace your lost or stolen U.S. passport, submit Form DS-64 with completed Form DS-11 to an Acceptance Facility

1. Fill out the DS-64 form here: DS-64

2. Follow the instruction for "New or First Time Passport"
